State safety officials have fined the city of Indianapolis $80,000 for an accident that killed a garbage truck worker. The Indiana Occupational Safety and Health Administration cited the city on Sept. 5 with a number of violations in connection with the death of Thomas Welch this April.

OSHA investigators stated that DPW workers routinely violated safety standards and the department didn’t “establish and maintain conditions of work which were reasonably safe and healthy employees”.

Welch was killed when a co-worker rolled over him with a garbage truck after mistakenly thinking Welch was riding on the right rear step of the vehicle. When the driver put the car in reverse, he heard Welch scream and realized he had run him over. He then stopped the truck and got out to see what had happened, after which point, he got back in the truck and moved the truck again to free Welch.
